A 23-year-old man comes to your ED after sustaining a right knee injury while playing soccer. You suspect a tear of the ACL. Which physical exam test is most accurate?
A recent 2019 study of ED patients aged 12-55 with knee injuries found that the Lever Test was 100% sensitive for ACL rupture. The Anterior Drawer/Lachman tests were only 40% sensitive. Diagnosis was confirmed using MRI of the knee as the gold standard.
